Climate change is increasingly being recognised not only as an environmental crisis but also as one of the greatest public health threats of the 21st century. Medical researchers warn that rising greenhouse gas emissions are exposing people to a growing range of health risks that extend far beyond heatstroke and respiratory illnesses. Evidence now suggests that prolonged exposure to climate-related hazards can affect nearly every major organ system, increasing the burden of chronic disease while reversing decades of progress in global public health.
Extreme heat, wildfire smoke, floods, worsening air pollution, changing disease patterns and declining food and water security are already affecting millions of people worldwide. While vulnerable groups such as children, older adults and people with pre-existing illnesses remain at the highest risk, scientists increasingly believe that repeated exposure to these climate stressors can have lasting health consequences for the wider population.
Among the body’s most vulnerable systems is the cardiovascular system. During periods of extreme heat, blood vessels widen to release heat, causing fluid loss through sweating and forcing the heart to pump significantly more blood to regulate body temperature. This increased workload, combined with dehydration, thickens the blood and places additional stress on the cardiovascular system.
In the short term, severe heat exposure can lead to heat exhaustion, heatstroke and heart failure. However, researchers warn that repeated exposure to heat waves over many years may contribute to long-term cardiovascular disease. Studies estimate that extreme heat is responsible for hundreds of additional cardiovascular deaths each year in the United States alone. Outdoor workers, low-income communities and individuals with limited access to cooling facilities are among those facing the greatest risks.
Scientists have also found that rising nighttime temperatures may be particularly harmful. Unlike daytime heat, warm nights prevent the human body from recovering from heat stress, increasing the likelihood of cardiovascular complications. Research from East Asia suggests that if greenhouse gas emissions continue to rise, hot nights alone could account for a substantial share of future heat-related deaths across the region by the end of the century.
The respiratory system is also under increasing pressure from climate change. Rising temperatures and prolonged drought have intensified wildfire activity across many parts of the world, exposing growing populations to smoke containing microscopic particles capable of penetrating deep into the lungs and entering the bloodstream.
Medical studies have linked wildfire smoke to inflammation, reduced lung function, asthma attacks and worsening chronic respiratory diseases. Smoke generated when fires burn through urban areas may be even more hazardous because it contains a mixture of toxic chemicals released from buildings, plastics and industrial materials.
Climate change is also contributing to worsening air quality by accelerating the formation of ground-level ozone, a pollutant that irritates the lungs. Longer growing seasons and rising temperatures have extended pollen seasons in many regions, increasing allergies and respiratory illnesses. Higher humidity and more frequent flooding further promote the growth of indoor mould, creating additional respiratory hazards inside homes and buildings.
Researchers are also uncovering growing evidence that climate change affects neurological health. Fine particulate matter from wildfire smoke has been associated with inflammation in the brain and may contribute to cognitive decline, dementia and stroke. Emerging research is also examining whether prenatal exposure to wildfire smoke could increase the risk of developmental disorders, although scientists stress that further investigation is needed.
Extreme heat itself can impair brain function. Studies have shown that high temperatures reduce concentration, lower academic performance and increase workplace errors. Older adults are especially vulnerable to confusion and cognitive impairment during prolonged heat events. Researchers have also found associations between unusually hot weather and higher rates of violent crime, psychiatric emergencies, suicide and worsening symptoms among people living with severe mental illnesses.
Climate change poses additional risks to reproductive health. Pregnant women exposed to extreme heat face a significantly higher likelihood of premature birth, while rising temperatures can worsen pregnancy-related hypertension and cardiovascular stress. Heat exposure has also been linked to reduced sperm quality and declining male fertility.
Changing climatic conditions are expanding the range of disease-carrying mosquitoes, increasing exposure to illnesses such as malaria, particularly among pregnant women whose immune systems are naturally altered during pregnancy. Rising sea levels are contaminating freshwater supplies with saltwater in some coastal regions, increasing the risks of hypertension, preeclampsia, miscarriage and premature birth.
The gastrointestinal system is likewise vulnerable to changing environmental conditions. Higher temperatures allow bacteria responsible for foodborne diseases to multiply more rapidly in food and coastal waters. Heavy rainfall and flooding frequently overwhelm sanitation infrastructure, contaminating drinking water with harmful pathogens and increasing outbreaks of diarrhoeal diseases.
Warming coastal waters have also created favourable conditions for dangerous marine bacteria, including Vibrio vulnificus, which can cause severe infections through contaminated seafood or contact with warm brackish water. At the same time, droughts and floods that destroy crops contribute to food shortages and malnutrition, weakening immune systems and leaving children particularly susceptible to intestinal diseases that can impair long-term growth and development.
Kidney health is another growing concern. Chronic dehydration caused by repeated exposure to extreme heat places sustained stress on the kidneys, leading to acute kidney injuries that may eventually progress into chronic kidney disease. Researchers have documented increasing cases of unexplained kidney disease among agricultural workers exposed to prolonged heat, despite the absence of traditional risk factors such as diabetes or hypertension.
Similar patterns have also emerged among migrant construction workers employed in extremely hot regions, where months or years of physically demanding outdoor labour have been linked to chronic kidney damage.
Scientists caution that many of these health impacts interact with one another. Heat, air pollution, food insecurity, infectious diseases and mental health challenges often occur simultaneously, creating cumulative risks that are greater than the effect of any single climate hazard.
Medical experts increasingly view climate change as a systemic health emergency rather than simply an environmental issue. As greenhouse gas emissions continue to warm the planet, the health burden is expected to grow unless countries strengthen climate mitigation efforts, improve public health preparedness and protect vulnerable communities from escalating environmental risks.
Researchers emphasise that reducing emissions, improving urban planning, expanding access to cooling infrastructure, strengthening healthcare systems and protecting ecosystems will be critical to limiting the long-term health consequences of a warming world. The growing body of scientific evidence underscores that climate change is no longer a distant threat but an immediate challenge affecting nearly every aspect of human health.
